Aim: To demonstrate the outcome of a bilateral perforating injury with IOFB from a chain saw.
Methods: Case report.
Results: A 14 year old boy was helping his grandfather preparing wood, when the chain saw broke. Each eye was hit by a tooth from the chain causing a perforating injury. No other injury was observed. Following primary wound closure and limited vitrectomy the patient had early vitrectomy bilaterally. An early secondary vitrectomy was performed in both eyes within 100 hrs. One eye needed further surgery due to a PVR development.
As in one eye the macula was hit and only the nasal retina could be preserved, this eye regained 20/600 vision only. The other eye recovered to 20/60 vision.
Conclusion: Due to early intervention, the grade of PVR could be reduced.
