Quality of care in orthopedic rehabilitation facing the shortening of stay

Recent changes in german health politics led through drastic cost reduction in health care. Especially post operative rehabilitation centers have to reduce the average time of stay continuously. Therefore we came up with the question if the high standard of quality in medical care can be kept facing the considerable reduction of average time stay.
In the Orthopedic Rehabilitation Center (Klinikum Neustadt) which is practicing Total Quality Management for years, a group of 115 in house rehabilitants from the year 1997 were retrospectively compared to a group of 220 rehabilitants of the year 2000. All of them had received a total knee replacement with PFC Sigma, DePuy in the Department of Orthopedics, Klinikum Neustadt in Holstein. Group one containing patients whose average total time stay was 37 days. Group two summed up patients whose average total time stay was 30,5 days.
Those Groups were compared in the Harris hip score related Staffelsteinscore and general satisfaction of the patient. It was shown that reduction of stay of 6,5 days does not lead to a significant reduction of the quality of health care. Further was shown that especialy the capacity increasement led to significant economic benefit.
