Background: Exercise blood pressure (ExBP) and BMI have an impact on cardiac alterations and therefore also an influence on cardiovascular morbitity and mortality. But it is unclear which of these parameters is of greater influence.
Methods: Out of 1466 patients a group of 557 (f: 317, m: 240, age: 48.7±11.7y.) was identified as normotensives after ABPM (daytime<135/<85mmHg, SpaceLabs 90207). All patients underwent standardized ergometry (50-100watts, 5 min recovery period), echocardiography and were finally arbitrarily divided into 3 groups according to their BMI (table 1 [Tab. 1]).
Results: Despite clear normotensive BP values during ABPM and comparable heart rates, ExBP has a great impact on cardiac alterations (LVMI, LA size, E/A ratio). This effect is more pronounced (G1ExBP-: G1ExBP+ = p<0.001; G2ExBP-: G2ExBP+ = p<0.001; G3ExBP-: G3ExBP+ = p<0.001) than the influence of BMI (G1ExBP-: G2ExBP-: G3ExBP- = n.s. and G1ExBP+: G2ExBP+: G3ExBP+ = n.s.).
Conclusions: Patients with normotensive blood pressure values during ABPM and increased exercise blood pressure showed typical hypertensive adaptations of the heart and this effect is significantly more pronounced (p< 0.001) than the influence of BMI.
