Abstract
A 9-year-old Egyptian male child presented on 21/9/2014 to E.R. department at Kasr AL-Ainy Teaching Hospital by left auricular abscess, patient underwent incision and drainage under general anesthesia, examination of the E.A.C and the drum cannot be done due to severe edema. Pus sample was sent for culture and sensitivity and till the result of the culture patient was on empirical antibiotics and frequent dressing. Culture was positive for pseudomonas and sensitive to Tienam(Impenim). The condition improved but not completely cured and the mother refused to continue medical treatment. After 2 months, patient presented to E.R. with recurrent auricular abscess, facial cellulitis, left partial facial palsy and deafness, urgent C.T ordered revealed left otomastoiditis and patient underwent incision and drainage again, culture and sensitivity done revealed also pseudomonas that sensitive to Tienam (Impenim) regarding the auricular abscess and left ear exploration done and cortical mastoidectomy done. Intraoperative, ossicles were eroded and granulation tissue was found, facial nerve was intact. Postoperative histopathology revealed inflammatory polyp. During admission, the wound healing was bad with no signs of inflammation, C.B.C done, revealed severe leucopenia, anemia and thrombocytosis. After pediatrician consultation, serum IgG level, Virology done revealed hypogloubinemia and HIV positive. Mother refused antiretroviral treatment and the patient died after 3 months.
