Objective

Herpetic keratitis is one of the most often reason for performing keratoplasty nowdays. Our data shows, that majority of patients with destructive corneal processes undergone keratoplasty were patients with herpetic eye disease - 68,8 %.

Aim

To compare objectivly clinical course of inflammatory process at rabbits after lamellar and penetrating keratoplasty performed concerning modeling stromal herpetic keratitis with ulceration.

Methods

Research was carried out on 30 rabbits.We worked out the modeles of primary and recurrent herpetic keratitis.All the animals used in the work were maintained with in accordance with the A.R.V.O. resolutions and European laws concerning the use of animals in the experimentation. For etiology confirmation we carried out PCR in conjunctive swabs and corneal tissue. In connection with that during supervision it was necessary to compare nonparametric attributes we have created a scale of an estimation of an inflammation. Statistical analysis was performed with the t test, Wilkokson and Mann-Whitney criterions.

Results

The comparison and the statistical analysis have shown, that the initial condition of modelled stromal herpetic keratitis in investigated groups directly ahead of operation performing - lamellar and penetrating keratoplasty, did not differ on any parameters of the estimation. The experimental researches have shown, that inflammatory reaction in the postoperative period was less expressed after lamellar keratoplasty, than after penetrating keratoplasty. Regress of the basic symptoms of an inflammation after lamellar keratoplasty came faster, than after penetrating keratoplasty It was marked considerably smaller - in 3 times - quantity of complications after lamellar keratoplasty (14,2 %), than after penetrating (43,75 %). Frequency of herpetic keratitis recurrences in the postoperative period after lamellar and penetrating keratoplasty had no difference.

Conclusions

The present study has showed, that lamellar keratoplasty should be perforemed in the cases of acute herpetic stromal ulcerative keratitis.
